EASE

SMS Opt-In Evidence

Real, unedited screenshots of the in-app opt-in screens described in EASE's A2P 10DLC campaign Message Flow. Consent is collected only inside the EASE and EASE Provider apps — there is no public webpage where this happens, so these screenshots are the verification evidence for that flow.

On every screen below, the consent checkbox/toggle is unchecked by default, optional, and separate from creating an account or signing in. The disclosure text reads: “I agree to receive text messages from EASE about my account, bookings, appointments, scheduling, cancellations, and coordination with my therapist. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for help.” — with Terms and Privacy Policy links directly beneath it.
